Permission to Travel Form for Minors


Both parents should sign a notarized consent to travel if their child is traveling internationally.  This requirement applies if you are married and leaving a spouse behind to work; grandparents taking grandchildren on vacation; same-sex couples with adopted children; a divorced parent on a trip to Europe, even a military family uniting for the holidays. 
The traveling parent or guardian should carry a notarized copy of the proper Children’s Consent Forms or Permission to Travel documentation.

These regulations apply to minors under 18 who are leaving the United States with school groups, teen tours, or friends on a vacation.

If you have sole custody, provide a copy of the relevant paperwork such as a death certificate or court order.



Specify the child’s full name and address, the names and addresses of both parents, the names, and addresses of the adults traveling with the child, the authorized destination countries and the dates of the trip.

Prisontalk- A Resource for Jail Inmate Relatives & Friends




PrisonTalk is an on-line information website designed to provide information and resources to families with incarcerated relatives or friends.  The concept of PrisonTalk Online web community was conceived in a prison cell and developed in a halfway house. 

The website is funded by donations from families of ex-offenders.  The site allows those with an interest in the prisoner support community a forum in where their issues and concerns may be addressed by others in similar situations.  

The website has a forum that allows a discussion of questions of public interest; a blog that features various noteworthy topics; a chatroom for topics on a specific subject, and an on-line store that monetarily supports prison families and prison reform efforts., etc.

How Can I Obtain the Signature of an Inmate on a Legal Document?

At various times, it is necessary to obtain the signature of an inmate on a legal document.  For example, a power of attorney form may be needed in the event the inmate wants to assign a friend or relative to handle business transaction/s.  

Other types of legal forms that require a notary are the Inability to Appear, Divorce Decree, Trust Deed or Health Care Directive forms.

A Mobile Notary can assist with obtaining the signature of an inmate detained in a detention center (jail).  


If you do not have the individual’s driver license or passport, the Notary Public can now accept the following identification: 1) An inmate identification card issued by the California Department of Corrections and Rehabilitation, if the inmate is in custody in California state prison; 2) Any form of inmate identification issued by a sheriff’s department, if the inmate is in custody in a local detention facility.
